As a Care Manager , you will provide post-transition services for participants either in their homes or in nursing facilities located within 9 miles of AHC .

Job Duties :
- Conducts comprehensive assessments and develops service plans as assigned
- Obtains nursing facility records
- Communicate outcome of assessments and next steps to class members and nursing facility staff
- Conducts wellness checks in accordance with consent decree contractual requirements as needed
- Documents all services using CCMPT policies, Medicaid 140, and accreditation standards
Job Requirements :
- Master’s Degree in social work, psychology, human services, or related field required
- 1+ year(s) of direct service experience with mental health or substance abuse for adults required
