Job Title : Licensed Practical Nurse (LPN) Memory Care Specialist

Location : Queensbury, NY

Shift : 15 : 00 - 23 : 30, 5 days per week (40 hours)

What You'll Do :

• Distribute medications with precision and care, ensuring each resident receives the treatment they need.
• Provide hands-on care and support to our memory care residents, enhancing their quality of life.
• Help assess and evaluate patients’ diverse needs, collaborating with our multidisciplinary team to achieve optimal outcomes.
• Contribute to the development and implementation of care plans and discharge plans tailored to individual needs.
• Facilitate and provide educational opportunities for patients and their families based on customized teaching plans.
• Adapt to the ever-evolving healthcare environment, embracing new technologies and processes with a positive attitude.
• Relay important patient changes to the registered nurse and multidisciplinary team to ensure seamless care.
• Manage and utilize department resources efficiently, supporting staffing needs and contributing to process improvement.
• Accurately document patient information in both electronic and paper medical records in a timely manner.
• Uphold HIPAA regulations and maintain confidentiality at all times.

What We’re Looking For :

• Qualifications : Licensed Practical Nurse (LPN) with Basic Life Support (BLS) certification.
• Experience : Prior experience in memory care or similar settings preferred.
• Skills : Strong clinical skills, effective communication, and a proactive approach to patient care.
• References : Two professional references required
