Key ResponsibilitiesEvaluate member needs and barriers to care, recommending personalized care plansCoordinate care between members, families, and providers to ensure appropriate servicesMonitor member progress and update care plans as necessary to meet goalsRequired QualificationsDegree from an Accredited School of Nursing or Bachelor's degree in Nursing2 - 4 years of related experience in care managementRegistered Nurse (RN) licensure requiredAbility to work remotely within the Central time zone
