Summary :
This position requires the skills and knowledge to providedirection, leadership and accountability for Medical Assistants.
Under general supervision, performs a wide variety of direct,technical nursing care functions for acute, sub-acute, convalescentand chronic patients in accordance with nursing care plans andphysicians' orders.
Referral Coordinator who is responsible forcoordinating all patient referrals, processes all requests forreferral authorizations.
Must be able to work cooperatively as ateam member and demonstrate behaviors of trust and respect towardothers including clinical support associates, non-clinical supportassociates, resident / faculty physicians, hospital associates andmanagement.
Responsibilities :
- As a Lead LVN - You will identify routine,urgent or emergency situations and follow procedures related toscheduling and obtaining proper referral / authorization fortreatment.
- As the Lead LVN- Greet all patientsas they arrive in a friendly, courteous, and professional manner;Demonstrate and maintain a working knowledge of customer serviceprinciples, and departmental expectations regarding customerservice.
Show concern for patient problems and ensure comfort ofthe patient while waiting to be registered or seen by the provider.
Document in the computer system all necessary demographic,insurance, and financial information. Compile or retrieve scheduledreports from computer system.
As the Lead LVN-Assist physicians by preparing patients for anexamination / treatment / procedure, assessing the patient's conditionby documenting the patient history, vital signs, chief complaints,etc.
Procedures may include but are not limited to, EKG, stresstests, casting, cast removal, suturing, Point of Care Testing, etc.
Performing care coordination activities with patients including butnot limited to chronic care management and transitional caremanagement services.
As the Lead LVN-Expedites the flow of authorization requests through the ManagedCare System. Prepares requests for authorization of services byensuring form completion, eligibility, verification, benefits, etc.
Accurately enter referral information into the computer system witha thorough understanding of the correct system codes (type, status,procedure, etc).
Performs care coordination activities, whichinclude but are not limited to notifying patients and providers ofauthorization decisions and maintaining accurate tracking ofservices including follow through on the status of referrals.
Coordinate the initiation of specific home health service, DMEservices diagnostics, etc.
Administerspecified medication (orally, intradermal, subcutaneous orintermuscular injection) and note time and amount in patient'schart.
Observe each patient and report adverse reaction tomedication or treatment to medical personnel in charge.
- Dress wounds, apply compresses, ice bags andhot water bottles. Assemble and use such equipment as catheters,and oxygen suppliers.
- Collect samples such asurine and sputum from patients for testing. Perform routinelaboratory tests on samples.
- Sterilizeequipment and supplies by following clinic policy. Maintainscleanliness of facility, exam rooms and instrumentation perpractice and OSHA policies.
Prepare exam rooms with necessaryinstruments.
- Order supplies per clinicalstandards.
- Document all incidents according tomandated rules and regulations and properly dispose of contaminatedand disposable items.
- As the Lead LVN- Leadthe Medical Assistants in each clinic with improving patient flow.Monitor workflow to ensure efficiency and timeliness.
Initiatestaffing changes and directing work tasks as necessary to managefluctuating volumes.
- Demonstrates competenceto perform assigned responsibilities in a manner that meets theage-specific and developmental needs of members served by thedepartment.
- Appropriately adapts to assignedassessment, treatment, and / or service methods to accommodate theunique physical, psychosocial, cultural, age specific and otherdevelopmental needs of each member served.
