We are seeking a Clinical Nurse who will work for the Pulmonary Adult Cystic Fibrosis Program Director. The position will primarily be focused on coordinating outpatient and inpatient care of adult CF patients and helping to coordinate the day-to-day activities of the Johns Hopkins Adult Cystic Fibrosis Program.
Except as noted below the job hours are weekdays during usual office hours.
Specific Duties & Responsibilities
Clinical Responsibilities 95% (Responsibilities will involve approximately 2 / 3 outpatient CF care, 1 / 3 inpatient CF care)
Outpatient
- Will work closely with 6 CF physicians and 2 experienced clinical nurses to provide care for patients of the Johns Hopkins Adult Cystic Fibrosis Program.
- Will be part of a multidisciplinary team that includes social workers, nutritionists, pharmacists, physical therapists, and research coordinators.
- Daily responsibilities will include aiding in all aspects of outpatient CF care, including sharing triage of patient phone calls, coordination of home care, follow up of lab and test results.
- Must be able to learn to independently triage patient calls, evaluate clinical data, and work closely with the physicians and other nurses to resolve patient issues and make medication and treatment changes, notifying pharmacies and patients of these changes.
- Must have outstanding phone communication skills to work with patients, physicians, nurse practitioners, pharmacies, equipment companies, homecare coordinators and insurance companies.
- Will follow up on treatment changes, prior authorizations, and equipment.
- Will teach patients about appropriate medicine use and CF treatment strategies.
- Will work in the outpatient clinic 2-3 days per week and in the Johns Hopkins Adult CF offices the other days of the week.
- Will work with the physician to manage patients’ IV home care, document progress and changes, and schedule clinic visits to determine when the IVs can be discontinued.
- Must have outstanding organizational skills to monitor patient lab results and microbiology susceptibility reports and notify patients of changes in care in response to these lab values.
- Will work with patients by teaching and identifying adherence issues with their CF treatment plans.
- Will develop a nursing plan of care to encourage increased education of patients and increased adherence of treatment plan.
- Will participate and help lead in Adult CF Program quality improvement initiatives.
- Will work with on-call infusion pharmacists and on-call CF attendings on weekends every third month to manage abnormal lab values for CF patients on home IV antibiotics.
This is done by phone from home, not in office.
Inpatient
Will rotate with other CF nurses every third week to provide direction and care for inpatient cystic fibrosis patients.
This will involve making daily rounds with the inpatient pulmonary team.
- Will interact with the inpatient care team to help coordinate new CF admissions.
- Will also work with the inpatient care team to optimize patient care by providing education and support of the transition from inpatient to outpatient care.
- Will provide ongoing education for the inpatient nursing staff to aid in maintaining inpatient CF expertise.
Research & Administrative Responsibilities 5%
- Will aid the Adult CF Research Coordinators in identifying appropriate patients for ongoing CF clinical trials.
- Will aid the CF Registry Coordinator with questions regarding recent patient history and clinical course.
- Other : Need to develop and maintain up-to-date expertise in cystic fibrosis by attending National CF Meetings with the Adult CF team, and by participating in ongoing CF educational programs.
- Any other job functions as determined by the CF Director.
Minimum Qualifications
Must be a registered nurse, licensed in the State of Maryland or state where practicing.
Preferred Qualifications
- Bachelor's Degree in Nursing or related discipline preferred.
- Minimum of two years of related nursing experience.
- Pulmonary medicine or program coordination experience.
- Must be able to independently assimilate and evaluate clinical data.
- A working knowledge of computers including Microsoft Office software and office equipment knowledge preferred.
