Role & Responsibilities* Work cooperatively with Northwest Health Services (NHS) Providers, supervisors, and personnel to carry out the goals and objectives of NHS according to governing body requirements and established internal policies and procedures.
- Treat others respectfully whether it is a patient, fellow employee, member of management, customer, vendor or visitor to our premises.
- Places patients in exam rooms and records vital signs, past medical history, present illness, blood pressure, weight, completes problem list, and prepares patient for Provider to see.
- Administers prescribed medications and treatments in accordance with nursing standards.* Assists patients to ambulate, assists patients onto / off exam tables and in / out of wheelchairs.
- Assists Providers in distribution of appropriate patient education materials and communicates information to the patient in a caring and professional manner.
- Triages patient calls and walk-in patients in a pleasant and timely manner. Consult with Provider whenever necessary in order to advise patient of most appropriate action;
reinforce Provider plan of care.* Determine, based on patient need, if and how quickly an appointment is needed for an assessment by a Provider and schedule accordingly.
- Provide education and assessment to patients looking to access Medication Assisted Treatment.* Assess and monitor patients in the induction, stabilization and maintenance phases of treatment.
- Provide ongoing management, education, and support of patients in all phases of treatment including but not limited to;
nursing visits scheduled and random, urine drug screens, routine labs and as needed, medication education, monitoring, film / pill counts, medication refills.
- Prepares patient record information for Provider referrals and communicates significant information to other health care Providers / entities as required* Assist in ongoing primary care of patients referred to the Medication Assisted Treatment Program.
- Work with the pharmacies and the M.A.T. team in obtaining medication orders, prescriptions, refills, prior authorizations and maintain ongoing communication as needed.
- Draws labs and administer injections as required.* Reviews charts for completeness for daily appointments. Documents all refills, telephone conversations, and patient office visits in the electronic patient medical record.
- Maintains tracking log for Hepatitis C patients and urine drug screens, both scheduled and random.* Attends and actively participates in clinical staff meetings, in-services, and educational programs, both at NHS and in the community* Documents all assessments, treatments, procedures and medications in medical record, electronic or paper during times of system downtime procedures.
- Collaborates, communicates and meets with MAT treatment team and other practice care team staff as needed and required.
- Complies with all regulatory agencies, directives, standards and regulations.* Perform other duties as assigned.QualificationsEducation & Experience* Graduate from accredited school, with L.
P.N. licensure in the state of Missouri.* BLS certification required.* Two years acute care nursing or two years office nursing experience preferred, with one year in substance abuse field preferred.
Possess light typing skills, computer knowledge required. Must demonstrate accuracy, proficiency, and efficiency.* Ability to visually assess patient condition and communicate with the patients and families with tact and professionalism.
Required Knowledge, Skills, and Abilities* Possess cognitive skills necessary to read and interpret a variety of instructions furnished in oral or written form.
- Must demonstrate tact patience and good communication skills in dealing with patients, co-workers and the public.* Must respect and maintain confidentiality at all times.
- Maintain a professional appearance and demeanor at all times.* Must be able to perform job requirements in high demand situations.
- Must be able to talk, see, hear, write, stand, walk, and kneel. Ability to move / lift patients in wheelchairs to exam tables or to push / pull equipment.
Must be able to lift a minimum of 50lbs.* Possess cognitive skills necessary to understand medical terminology, instructions, medical records, and identifying problems and offering solutions.
- Follow established procedures for answering and screening incoming appointment-related telephone calls, scheduling interpreters, and directing calls to appropriate staff.
