facilitating interdisciplinary approaches, coordinating and providing care that is safe, timely, effective, efficient, and patient-centered.

Required Qualifications : Must possess the skill, knowledge and ability essential to the successful performance of assigned duties Must demonstrate customer service skills appropriate to the job Excellent written and verbal communication skills in English Ability to maintain a work pace appropriate to the workload Ability to establish and maintain effective working relationships across the Health System Minimum Education : Current California RN license Minimum Experience : Three to five years of experience in an acute care hospital setting (medical / surgical preferred) Previous one to two years of experience as a Case Manager.

Must be proficient in typing. Ability to write and communicate professionally Must be proficient in computer skills including Microsoft Office Preferred Qualifications : Knowledge of University and medical center organizations, policies, procedures and forms Broad knowledge of Medicare, Medi-Cal and insurance guidelines.
