Position Summary
The primary responsibility of your position is to provide care to the Elders living in a Green House home by protecting, sustaining and nurturing the Elders, providing assistance with activities of daily living and meeting other needs as required.
The core goal of the Shahbaz’s work is to provide assistance in a manner that promotes a high quality of life and a positive Elderhood.
All responsibilities will follow the current federal, state and local standards, guidelines and regulations that govern long term care and assisted living facilities.
You are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.
Essential Duties
- Working with your household team of Shahbaz, recognize and respond to the needs and desires of the Elders
- Promote autonomy of Elders in decision-making regarding their daily lives and the life of the house
- Work together with the other Shahbaz as a self-managed team, responsible for managing all aspects of the home, including food ordering, preparation and clean up, light housekeeping duties, all laundry needs of the elders, and assisting Elders in engaging in the daily activities of their choosing
- Cooperate with the nurses and other clinical team members to ensure the well-being of each elder
- Receive and report resident status at the beginning and end of each shift and as needed to team members and to nurses and clinical support team members
- Assist Elders with personal care functions, including : dental and mouth care, bathing, dressing and grooming, hair care, nail care, shaving, bowel and bladder care, other personal and hygiene care
- Assist with and / or perform restorative and rehabilitative procedures as outlined and assigned by the care plan
- Measure and record vital signs such as temperature, pulse, respirations, weight, height, and anatomical dimension / circumference
- Assist moving, positioning and transporting Elders into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.; make room checks and verify resident location
- Assist with the application of slings, elastic bandages, binders, etc
- Provide range of motion exercises as outlined and assigned by the care plan
- Work with the Elders and the Dietitian to create a balanced menu that meets the nutritional needs of the Elders as well as their preferences for foods they enjoy
- Prepare and serve meals, between meal, bedtime and other nutritional snacks per the posted menu and stated preferences of the Elders
- Keep all areas of the home clean, performing routine housekeeping duties ( changing bed linens, emptying trash, dusting)
- Launders clothing, linen and other articles
- Assures Elder calls for assistance are answered promptly
- Cooperate with the Activity professional to develop and carry out support of the Elder’s daily activities, including opportunity for spontaneous activities to occur
- Maintain confidentiality and rights of Elder, family, employee, and house information in compliance with PHI HIPAA policies and applicable regulations
- Meets with the Clinical Service Team (CST) as schedule or needed to discuss Elders’ needs and progress in identifying and correcting problems
- Observe, monitor, and report to the CST any change in condition
- Document observed data per policy
- Will serve in different coordinator roles on a rotating basis. These include Team, Care, Scheduling, Food & Housekeeping Coordinator
- Attend and participate in Team meetings on a regular basis
- Attends and participates in orientation programs, on-going training and educational classes
- Maintains all equipment and appliances in the house by adhering to proper operating guidelines
- Documents and maintains records necessary to comply with all state and federal regulations
- Report any problems with equipment or appliances immediately to the maintenance director
- Adheres to all safety guidelines and processes; and observe, monitor, and intervene in or report to the Guide any unsafe conditions;
observes standard precautions
- Reports accidents and incidents when they occur
- Report any abuse or suspected abuse to the Guide immediately
- Follows established smoking regulations and reports violations. (Non-smoking campus)
- Must be willing to learn and perform duties beyond the traditional scope of practice of a CNA, such as cooking, cleaning, laundry and activities
- Must be able to adhere to the core value and practices of a Green House Home
- Perform all other duties as requested
Qualifications
Basic Qualifications & Experience
- Must possess a current, unencumbered, active CNA certification to practice in the state
- Six months experience in a long-term care environment preferred
- Must be able to read, write and speak the English language.
- Must possess excellent customer service skills.
Working Conditions / Physical Requirements
- Position requires medium-heavy physical work. Heavy lifting, pushing, or pulling objects up to 100 pounds of force occasionally and / or up more than 45 pounds of force repeatedly to move objects.
- Stands and walks constantly during working hours with occasional sitting, kneeling, squatting, and reaching above shoulder height and fine motor control with fingers.
Also requires grasping.
