RN - Care Manager

Location : Santa Monica, CA

Virtus Healthcare Staffing is seeking a dedicated Registered Nurse (RN) to join our partner Health Center in Santa Monica, CA, as a Care Manager.

In this role, you will play a critical role in coordinating and facilitating patient care, ensuring smooth transitions, and promoting positive outcomes.

Responsibilities :

Care Coordination and Advocacy : Coordinate and advocate for the complex care needs of patients, ensuring a smooth transition through the healthcare system.

Assessment and Planning : Conduct comprehensive assessments of patient needs, develop individualized care plans, and collaborate with the multidisciplinary team to implement effective interventions.

Resource Management : Identify and secure appropriate resources and services for patients, navigating insurance complexities and advocating for optimal care access.

Discharge Planning : Facilitate timely and safe discharges, ensuring patients have the necessary support and resources to continue care at home.

Communication and Education : Provide clear and compassionate communication to patients, families, and healthcare providers, educating them on complex medical information and treatment options.

Quality Improvement : Participate in performance improvement initiatives, utilizing data and evidence to drive positive change and enhance patient outcomes.

Required Qualifications :

Education :

Bachelor's Degree in Nursing OR equivalent education / experience

Master's Degree in Nursing or Healthcare related field preferred.

RNs with 20+ years of experience are not required to obtain a BSN.

Any previously agreed upon effective date for obtaining a Bachelor's degree in nursing between the individual and the facility will remain in effect.

Any collective bargaining contract with specific time frames for obtaining a Bachelor's degree in nursing will supersede the above point and will remain in effect.

Exceptions to the BSN requirement will be considered on a case-by-case basis.

Licensure :

Upon hire : California Registered Nurse License

Experience :

2 years of experience in a healthcare-related field (Acute, Ambulatory, Post-Acute, etc.)

2 years of experience in Case Management (Care Coordination or Utilization Management) OR successful completion of the Transitions in Practice (TIP) program for Care Managers.

TIP candidates must have experience in the same type of nursing unit where the CM position is available.

Preferred Qualifications :

Upon hire : CCM (Certified Case Manager) or ACM (Accredited Case Manager) certification
