The Key Program is seeking a Service Coordinator for the Family Networks Program in New Bedford, MA . The Service Coordinator will work directly with the Department of Children and Families (DCF), within the DCF regional office.

The Family Networks Service Coordinator requires a Bachelor’s Degree in related human service / clinical field and one (1) year of case management or direct care experience.
Bilingual / bicultural applicants are strongly encouraged to apply. All positions require applicants who are at least 21 years of age or older and have a car and valid driver’s license for work use.
What you will do
As the Family Networks Service Coordinator , you are responsible for the coordination and case management of services between families and providers to ensure the appropriate level of placement, to include :
- Responsible for the coordination and case management of services between families and providers to ensure the appropriate level of placement.
- Conducts intakes, interviews and assesses clients in preparation for referral to the network.
- Coordinates and facilitates treatment plan review meetings, initial diagnostic conferences and discharge planning meetings.
- Ensures appropriate service implementation and case coordination.
- Works with providers to ensure that client documentation, billing and data entry is accurate and timely.
