Duties of the CNA / HHA may include :
Follow the POC for each client and document the same
Help clients to maintain good personal hygiene
Assist in maintaining a healthful, safe environment
Assist clients with ambulation as per the POC
Assist with certain treatments as ordered by the physician / approved and supervised by the nurse
Assist the therapy personnel as needed with rehabilitative processes per the HHA POC
Taking and charting vital signs under the direction of a nurse or healthcare practitioner
Encourages clients to be as independent as possible according to the nursing care plan
Attempts to promote client’s mental alertness through involvement in activities
Establishes a relationship with client and family which transmits trust and confidentiality
Reports any changes in the client’s status or home situation immediately to the supervisor, staff nurse, or aide supervisor, including recognizing & reporting skin changes
An active participant on the multidisciplinary team and QA program
Performs routine housekeeping tasks as related to a safe and comfortable environment for the client, as instructed by the nurse
Attends in-service as required by state and federal regulation
Other duties as assigned
Qualifications
Must one of the following requirements :
Satisfactory completed a training & competency eval program that complies with the requirements of TX Code and CMS regulation (minimum 75 hours) and have a certificate of completion or
Satisfactory completed a competency eval program that complies with the TX Code and CMS regulation or
Successfully completed a nurse aide training / competency eval program approved by the state of TX as meeting the CMS and be listed on the TX DHS’s nurse aide registry in good standing, with no findings relating to abuse, neglect or misappropriation of client property or
Meets the requirements of a TX state licensure program that meets the provisions set forth by CMS Conditions of Participation for Home Health Agencies for Home Health Aide training & competency.
As well as :
Has provided home health services within the previous 24 months.
And :
Prefer a high school graduate or equivalent
The necessary skills to perform their job responsibilities
Is registered with the TX Department of Human Services’ (DHS’s) NAR with no findings of client abuse or neglect or misappropriation of client’s property.
The ability to read, write and communicate effectively in English, consistent with job requirements
Reliable means of transportation to get to assigned client visits
Must clear a criminal background & other required checks and current CPR certification.
