Job Description :
- Join our Healthcare team as a Clinical Social Worker, providing clinical social work, assessments, therapy, and discharge planning.

- Must have active LCSW license, 12 months experience, and BLS / CPR certification.

Responsibilities :
- Perform clinical social work with individuals with diverse needs.
- Conduct assessments and develop treatment plans.
- Provide individual and group therapy as outlined in the treatment plan.
- Coordinate discharge planning and access appropriate community support and services for patients.
Qualifications :
- Licenses Required : Current Licensed Clinical Social Worker (L.C.S.W.)
- Minimum of 12 months experience
- BLS / CPR Certification.
- Copy of Negative TB test / Chest X-ray.
- Familiarity with EMR / EHR systems.
