Per Diem Registered Nurse Case Manager

Anticipated Territory : Santa Clara County : San Jose*

Schedule : Saturday-Sunday, Saturday-Monday, or Friday-Sunday

What you’ll do :

Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including :

• Conduct Start of Care (SOC) Admissions - OASIS
• Conduct follow ups, evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients’ conditions, reporting changes to Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.

To be a success :

• Current and unrestricted CA RN license
• Current BLS / CPR Certification
• Valid CA Driver's License
• Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired
• High level of clinical knowledge in home health care is highly preferred
• Experience with high acuity patients is a plus
• Experience working with an interdisciplinary team is highly desired
• Case Management fundamentals is highly desired

• You will be assigned to a primary location, however you will be expected to be available to cover for vacancies, leave of absences, as well as supporting increased census and client care needs outside of the assigned area.
