Essential Job Functions Include :
- Evaluate new and readmitted Residents for rehabilitation potential and make written recommendations.
- Develop effective treatment plans for Residents in accordance with physician orders.
- Provides care that follows treatment plan and ensures Residents are free from abuse, neglect, or mistreatment.
- Complete all documentation and assessments in accordance with all policies and regulations.
- Assist with Resident plan of care and attend meetings in regards to admission and discharge.
- Participates in Resident, family, and staff education as needed.
- Assist with cleaning of treatment and department area.
- Additional duties as assigned by the Director of Rehabilitation.
Qualifications :
- Must be a graduate from an accredited PT school with a BS in Speech Therapy.
- Must possess a current NYS License for Speech Therapy.
- Experience working with the elderly and / or in a health care setting is desirable.
- Must have knowledge of Therapy procedures.
- Must be able to advocate for seniors and families.
- Must be available for Holidays.
- Be a team player and work cooperatively with all staff in the facility.
- Must be able to read, write, speak and understand the English language in an understandable manner.
- Must be able to perform essential job functions (reasonable accommodation may apply.)
- Must be able to function independently, have flexibility, personal integrity and the ability to work effectively with Residents, staff, visitors, volunteers, government agencies / personnel, etc.
