BC / BE Palliative Care Physician to expand an existing team in the Palliative Care Center.
The Center’s focus is to provide and coordinate palliative care across the entire spectrum of patients, including oncology, cardiology, neurology and nephrology.
Active education about palliative care across the medical community
Coordination with the Hospice Program with potential for integrated coverage
Will join a Board certified Palliative Care physician to provide care primarily in an outpatient clinic setting
Includes inpatient consults at the Hospital where a majority of inpatients are treated by an employed hospitalist group;
current Palliative Care physician serves as Director, Palliative Care Services
Supported by a multidisciplinary team including an advanced nurse practitioner, nurse clinician, social workers, chaplains and other allied health personnel
Call one weekend per month and alternating with current physician during the week
