The Mental Health Counselor is responsible for providing psychosocial assessments, collaborative treatment planning, engagement, referrals and evidenced-based clinical interventions as indicated for individuals dealing with a variety of mental health and co-occurring substance use issues.
They will display evidenced of excellent organization, flexibility, time management, initiative and communication skills required to provide therapy within an integrated community health setting.
As part of the Essential Function for this role, the Mental Health Counselor :
- Conducts initial psychosocial assessments and provides initial treatment recommendations and collaboratively with the patient develops a personalized treatment plan with emphasis on evidenced- based practice within a harm reduction orientation.
- Provides resources and referrals as indicated to facilitate achievement of a patient’s treatment goals
- Provides psychotherapy and crisis intervention to individuals and families who are dealing with a variety of issues including but not limited to mental health, co-occurring substance use disorders, chronic medical conditions including HIV, HCV and issues related to sexual orientation and gender identity.
- Adheres to program expectations and requirements surrounding productivity, documentation timelines and standards, and overall quality of patient care.
- Works within an integrated treatment model coordinating treatment with both internal and external providers as indicated.
- Will serve as a role model for positive, productive, and cooperative relationships with staff and clients.
- Adheres to Office of Mental Health regulations and Evergreen Health policy and procedures including regulatory timeframes on clinical documentation using an electronic medical record system
- Maintains confidentiality of all treatment records.
- Attends and participates in regularly scheduled mental health and agency staff meetings, trainings, case conferences and individual and group supervisions.
- Other duties as requested.
Qualified Candidate will have a Masters of Social Work or other related field with two (2) years of related experience. LMSW or LCSW preferred.
Candidate must be able to communicate clearly and professionally in writing and verbally, demonstrate familiarity with computers and MS Office software (Outlook, Word) and effective time management and organization skills.
Sensitivity to people living with HIV / AIDS, addiction, cultural and LGBT issues essential.
Job Type : PART TIME or FULL TIME
Required Education : Master’s degree in social work (or similar), two (2) years of relevant experience, and LMSW / LCSW preferred,
Required Experience : Working knowledge utilizing electronic medical records software (Medent) strongly preferred. Knowledge of Diagnostic and Statistical Manual of Mental Disorders.
