Your Role With Us :

As a Physical Therapist, you'll work in collaboration with Orthopedic Surgeons and Advanced Practice Providers (APPs), ensuring a seamless care experience for our patients.

Our primary focus is on helping those recovering from surgery reclaim their health and improve their quality of life. At Salem Health's State Street Outpatient Rehab clinic, you'll provide exceptional patient care by using evidence-based practices.

You will maintain strong communication with patients and staff, efficiently manage patient care coordination, and work in a supportive environment.

Key Responsibilities :

  • Patient Care : Develop and implement treatment plans, ensuring progress towards functional goals with safe, effective strategies.
  • Documentation : Maintain accurate documentation and billing, reflecting patient progress and supporting charges.
  • Care Coordination : Work collaboratively with the healthcare team to maximize patient outcomes and involve families in care plans.
  • Caseload Management : Proactively manage caseloads to meet productivity targets, with flexibility required for department needs.
  • Post-Operative Care : Previous experience in post-operative care is preferred.
  • Communication and Compliance : Maintain effective communication and adhere to regulatory standards.

Background / Expertise :

  • BS, MS, or Doctorate in Physical Therapy required.
  • Basic Life Support (BLS) required within 60 days of hire.
  • Licensed Physical Therapist in the State of Oregon required.
  • Post OP Care Experience preferred.
  • Be a team player and have proven success applying team approach to obtain resolution to an issue.
  • Excellent interpersonal communication skills, problem solving, decision making and organizational skills for working with the rehabilitation team and patients.
  • Ability to prioritize and multi task while working in a high volume multidisciplinary setting.
  • Demonstrated competency by implementing age, culturally sensitive, and developmentally appropriate treatment plans and by appropriate documentation.
