Typical shift is 10-7, with rotation of 12p-9p shifts, and rotating weekends

Pay Range $48.00-$60.97 Per Hour

Responsibilities : As an Admissions Registered Nurse at Snowline Hospice, you will be responsible for providing compassionate and comprehensive nursing care to our patients and their families.

Your duties will include :

• To coordinate and provide care to our hospice patients
• To calm, soothe and empathize with the strong emotions our patients and families are experiencing
• To participate with a team of healthcare professionals in superior level of care for each patient
• To address personal care needs and promote patients’ comfort needs
• Collaborate on cases, involvement in interdisciplinary meetings, training and development and other organizational activities
• Assist with observing the patient along with documenting care
• The Admissions Registered Nurse (RN) makes patient referral visits and facilitates patient admission to hospice.
• Develop a hospice presence in hospitals, SNFs, RCFEs with possibility of attending rounds and care conferences, conferring with providers, speaking at in-service meetings, attending committee meetings, and participating in new staff orientation
• Act as a liaison and advocate for Hospice patients admitted to the hospital, SNFs, and RCFEs
• Assess each consult to determine the appropriated for Hospice admission, based on individual Hospice criteria
• In close collaboration with other disciplinary teams, you will be actively supporting quality care and assist in improving the lives of others
• Enjoy meaningful interactions as you join forces to make good things happen
• Empower, educate and coordinate all care details of your hospice patients
• As an RN you calm, soothe and empathize with the strong emotions our patients and families are experiencing
• To motivate and encourage a team of healthcare professionals in providing the best care possible to each and every patient
• To address pain, comfort and manage symptoms in heroic ways
• Collaborate on cases, involvement in interdisciplinary meetings, training and development and other organizational activities
• As a registered nurse, you assist with the management of clinical notes, health records, and systems to ensure appropriate and accurate data
• Comfort driving in inclement weather and in remote parts of El Dorado County

Requirements

Your Experience and Education :

• BSN, Associate Nursing Degree, or a diploma from an accredited school of nursing
• Valid California Registered Nursing License
• Minimum two years experience in nursing (one year hospice experience preferred)
• Self-directed and able to work with minimal supervision, ability to organize and prioritize workload independently
• Excellent observation, problem solving, verbal and written communication skills
• Demonstrated ability to efficiently use a computer, prior electronic medical record (EMR) experience preferred
• Current BLS / CPR certification
• Valid CA driver’s license, with a reliable vehicle that is insured in accordance with state requirements
• Is open to an occasional flexible schedule to work evenings or weekends
• Is comfortable being mobile and can travel weekly throughout the Placerville and surrounding territory
