Purpose :
To perform direct care duties under the supervision of licensed nursing personnel and to assist in maintaining a positive physical, social and psychological environment for the residents.
Qualifications :
Must Be a Certified Nursing Assistant in good standing and meet all applicable federal and state certification requirements or training to satisfactory complete the requirements or in training to satisfactory complete the requirements to become a Certified Nursing Assistant in accordance with state and federal guidelines.
Must be 18 years of age
Completion of high school or GED equivalent or otherwise show the ability to read and write and follow oral commands in English
Previous experience or completion of an appropriate training course
Positive attitude toward elderly
Must be eligible for clearance through abuse registry and Department of Law enforcement.
Essential Job Functions :
Personal Care Functions
Assist residents with bathing, dressing, grooming, dental care, bowel and bladder functions, preparation for medical tests and exams, ear and eye care and positioning in and out of beds, chairs, etc.
Performs patient related activities directed throughout the shift including assisting with lifts of patients to mats, tilt tables, & wheelchairs, assists patients in transfer activities, assists patients in dinning & removing appliances, and guards’ patients in ambulation.
Always performs these functions in a constant state of alertness and in a safe manner.
Nursing Care Functions
Provides nursing-related functions as directed by Nurse Manager including daily perineal care, catheter care, turn residents in bed, sponge baths, measure and record temperature, pulse and respirations, weigh and measure residents, perform restorative and rehabilitative procedures and observe and report presence of skin changes;
review care plan daily, report changes in resident conditions to supervisor; and record all necessary charting entries and reports all accidents and incidents.
Follows proper charging procedures for resident care items; cares for the dying resident and provides post mortem care.
Always performs these functions in a constant state of alertness and in a safe manner.
Resident's Right Functions
- Maintains resident confidentiality and privacy;
- Treats residents with kindness, dignity and respect;
Knows and complies with Resident's Rights under federal and State law and regulation, including HIPAA / HITECH; and,
Promptly reports all resident complaints and incidents to supervisor.

Support Functions
Assist as directed in proper admission, transfer and discharge of residents' inventory, resident possessions and report food articles and medications if found in resident's room and report defective equipment to administration.
Safety and Sanitation Functions
- Understand and use Universal Precautions and adheres to strict infection control guidelines including hand washing, hazardous communications and other safety rules;
- ensure cleanliness of assigned residents' rooms; properly maintain and record resident restraints; promptly report all exposure to infection in accordance with OSHA and Avante guidelines, policies, procedures and all violations of safety and sanitation rules to supervisor;
- Demonstrates knowledge of emergency policy and procedure;
Performs all duties in a safe and efficient manner; including placing residents' call light within easy reach, raising bed rail(s) when resident is in bed if appropriate.
Follows established guidelines when using physical restraints / protective devices and equipment.
Demonstrates awareness of potential hazards and reports unsafe conditions to appropriate facility staff to maintain a safe environment for residents, families and staff (i.
e. reports spills, faulty equipment, maintains a clear path for resident ambulation, uses wheel chair locks appropriately, etc.).
Pick up all equipment as needed (generally up to 25 lbs).
Cleans and repairs wheelchairs as needed.
Puts supplies in closet when received.
Administrative Functions
Answers telephones, takes and delivers messages throughout shift.
Maintains inventory of supply closets ongoing.
Attend and participate in orientation, training, educational activities and staff meetings mandated by facility, state and federal guidelines.
Staff Development Functions
Responsible for pursing and completing a minimum of 12 hours in service education per year where individual development is needed to comply with federal guidelines.
Serves as a preceptor to other Nursing staff when called upon.

Education and Experience :
- High School Diploma or equivalent required;
- Certified Nursing Assistant License in good standing with the State required;
Previous work experience in long term care industry strongly preferred.
