SUMMARY OF POSITION n n : n

n The Intensive Case Manager will assist clients enrolled in the Opioid Treatment Program with accessing community based resources, housing, and employment;

make appropriate referrals and perform general case management and counseling. n

n n n MINIMUM QUALIFICATION(S) : n n n n n

• n High School Diploma or equivalent required. n
• n Act 31 training completion is required within 45 days of hire. n n n

n n n PREFERRED QUALIFICATION(S) : n n n

• n Two (2) years experience in a related human service field preferred. n
• n One year as a Drug and Alcohol Case Management Specialist Trainee. n
