What You'll Do :
- Patient Care : Provide skilled nursing care to patients in their homes, following established care plans.
- Administer Medications : Safely administer medications and treatments as prescribed by hysicians.
- Monitor Health : Track and document patient progress, reporting changes to the care team.
- Educate & Support : Educate patients and families on healthcare needs, providing emotional support.
What we are looking for :
- Licensed Vocational Nurse (LVN) : Active LVN license in California.
- Experience : Minimum of 1 year of nursing experience preferred, home health experience is a plus.
- Compassionate & Reliable : A caring attitude with a commitment to providing the best care possible.
- Strong Communicator : Ability to communicate effectively with patients, families, and the healthcare team.
- Flexible : Willingness to work various shifts, including some weekends and holidays.
