job_description.job_card.job_descriptionThe Physical Therapist II assesses individual patients' Physical Therapy needs and establishes and implements a treatment program. Reviews medical history, applies screening tests, and meets with patient and family to determine clinical needs, impairments, functional limitations, and disabilities. Determines a plan of care including appropriate interventions, expected intensity, duration of therapy, and discharge recommendations. Provides education to patient, family, and / or caregiver regarding activity purpose, equipment use, home exercise, and activity. Performs regular re-examinations, measures outcomes, and seeks feedback from patient, family, and interdisciplinary medical team. Oversees clinical work of Physical Therapist Assistant and Rehabilitation Aide. Manages multiple patients simultaneously and provides care in multiple clinical settings and locations. Provides timely and accurate services and documentation in accordance with professional and regulatory standards. Participates in continuing education and incorporates new information into practice. Assists with educating and orienting new staff, acts as clinical instructor for students and interns, and participates as instructor in established classes or programs. Participates in educational events within the clinic and community. Attends clinical education coursework meeting Physical Therapy Board of California guidelines and provides in-services to staff. Eighteen (18) months of experience as a Physical Therapist in an acute healthcare system environment. Training and experience in completing internal pelvic floor exams and treatment. Competent in treating all male and female adult pelvic health conditions. Demonstrates competency in Inpatient Rehabilitation Facility Quality Reporting Program (IRF QRP). May require travel to patient’s home for home evaluation. Experience with a range of diagnoses including general medicine, orthopedics, neurology, and cardiopulmonary. Fulfills mandatory stroke education requirements per certification agency. Experience with internal pelvic floor exams and treatment. Competent in adult pelvic health conditions. Experience in inpatient rehabilitation and acute healthcare settings. Fulfills mandatory stroke education requirements per certification agency. Preferred experience with Microsoft Office and healthcare database systems. Bilingual or multilingual.
