About The Position
Weekly Pay Range - $1836 To $2040 Job Title : Physical Therapist Job Specialty : Med / Surg RN Job Duration : 13 weeks Shift : 5x8.
0 (Monday - Friday, 8 : 00 am - 4 : 30 pm; weekends as needed) Guaranteed Hours : Not specified Experience : Minimum of one year experience in Long Term Care / Home Care setting as a Physical Therapist.
Recent Home Care experience (within the last year) strongly preferred. License : Current state Physical Therapy License Certifications : Not specified Must-Have : Ability to physically lift and assist patients in hands-on care.

Job Description : Provide case management and direct patient care within a team environment.- Administer comprehensive physical therapy modalities in the home setting.
- Assist the physician in evaluating the level of function and help develop the plan of care.- Prepare clinical and progress notes and consult with the family and other personnel.
- Participate in in-service programs and work collaboratively with various stakeholders.- Ensure care delivery services align with the agency's vision, mission, values, and strategic goals.
- Promote effective communication and teamwork to support organizational goals.- Manage resources efficiently to contain costs and enhance service value.
- Develop and revise treatment plans and discharge plans.- Initiate and supervise appropriate rehabilitative programs.- Provide education on disease processes, self-care techniques, and preventive strategies.
- Implement home therapy programs, including gait training and the use of equipment.- Periodically evaluate patient's progress and adjust therapy programs as necessary.
- Instruct para-professional staff in care techniques and ensure quality patient care services.- Participate in regular patient conferences and assist in the orientation of new personnel.
- Maintain compliance with state and federal regulations and documentation requirements.- Contribute to continuous improvement efforts and promote recognition and respect for team diversity.
- Participate in ongoing education, professional organizations, and sharing knowledge with team members.- Monitor quality indicators and strive for quality and cost-effective outcomes.
