Ensure clients receive appropriate brain injury services based on assessed client needs, and support the overall treatment component of the program.

Consult routinely with the Brain Association of Pennsylvania to coordinate brain injury interdisciplinary team approach to support the clients.

Maintain quality assurance of initial assessments and its relationship to the development of ISP. Conduct clinical reviews, develop and implement evidence based, brain injury in-service training component and staff development program for all staff based on current trends or client needs.

SCOPE OF DUTIES :

  • Meet minimally one time per week or more often if needed with treatment team to review individual service plans and provide feedback and input into client’s individual development plans.
  • Develop and implement evidence based, brain injury in-service training component and staff development program for all staff based on current trends or client needs.
  • Consult with BIAPA to discuss clinical issues.
  • Ensure consumer medical needs are identified and services are consistent with the individual service plan.
  • Ensure the provision of direct services to all program participants daily.
  • Quality assurance of initial assessments and its relationship to the development of ISP for all consumers.
  • Ensure appropriate brain injury services and resources are available and accessible.
  • Works towards attaining outcomes and mentors direct care staff in their delivery of individual interventions.
  • Provide assistance with quality assurance efforts to ensure program integrity.
  • Oversee all HIPPA related procedures ensuring the Neurocognitive Disorder Program remains compliant.
  • Conduct clinical reviews with Clinical Specialist at least one time per month.
  • Collect data to support research initiatives and service delivery.
  • Provide on-going feedback to treatment team with regard to brain injury program initiatives, program philosophy, models and approaches.
  • Maintain professional and technical knowledge of local, state and federal legislation and regulations and respond in an approved manner to issues that have potential impact on the program and client.
  • Maintain 24 hours of annual training as it relates to brain injury.
  • Assist primary health care providers in recognizing the underlying brain injury, and treating mental disorders and psychosocial problems.
  • Assist in the detection of at risk clients and development of individualized plans to prevent further psychological and physical deterioration.
  • Assist in preventing relapse or morbidity in conditions that tend to recur over time.
  • Provide brain injury education to participants, families and staff about care, prevention and treatment.
  • Evaluate clients’ care with the primary care team.
  • Network with community agencies to develop alternate resources for brain injury treatment support and aftercare.
  • Interact and communicate (verbally and non-verbally) with clients, staff and outside contacts professionally with respect and courtesy.
  • Maintain client and staff confidentiality in compliance with administrative policies and procedures.
  • Attend and participate in regularly scheduled staff meetings, supervisory conferences, training sessions and liaison with families, friends, advocates, external agencies and management team to provide consistent brain injury approach to meet client’s individual needs.
  • Other duties as assigned.

ADA ESSENTIAL FUNCTIONS :

  • Frequently required to sit in meetings and documentation of required paperwork.
  • Frequently required to talk and hear communicating with others exchanging accurate relevant information.
  • Frequently required to see and observe beyond arms reach seeing up and down or to the left and right observing surroundings.
  • Frequently required to use hands to handle or feel writing, operating computer and other office equipment.
