Job Description
We are looking for a Program Supervisor for the Day Hab. Programs in Brooklyn and Manhattan . Under the direction of the Director, the Program Supervisor facilitates Person Centered Planning and coordinates individualized services.
The supervisor supervises the Direct Support Professionals (DSP), ensuring effective service provision. Promotes independence and community inclusion of individuals served through creative planning, effective use of staff management and professional teamwork.
Develops individualized plans with DSP and ensures compliance with New York State Home and Community-Based Waiver Services regulations, agency philosophies, and policies.

SALARY : Bachelor $47.3K, Master $49.3K SCHEDULE : Monday-Friday 8 : 30am-4 : 30pm
General Responsibilities :
- Evaluates assigned support staff on an ongoing basis implementing a job performance evaluation annually; develops appropriate goals for staff and follow through on staff progress.
- Assists in hiring and training process for Direct Support Professionals; supervises staff on an ongoing basis either individually or in small groups.
- Ensures that individualized habilitation plans are consistent with individuals’ service plans and are supported by the team;
follows through on each individual’s established outcomes and documentation of progress by the Direct Support Professionals.
- Facilitates and supports the development and maintenance of individual support networks through outreach to family members, community members, and professionals and through coordination of personal planning sessions in collaboration with the individual served.
- Assigns staff various duties particularly in the case of staff absences; ensures that there is adequate supervision for the service needs of the individuals served at the program.
- Functions as liaison between Service Coordination Services and Day Habilitation service.
- Participates in any special case conference, staff and parent meetings as appropriate.
- Monitors safety issues, coordinates, documents medical supports, practices appropriate emergency procedures, responds to emergencies and promotes individual rights regarding health and safety issues.
- Ensures that program remains in compliance with New York State Home and Community-Based Waiver Services regulations, agency philosophies, and policies.
Qualifications
- Associate's Degree required. BA / BS preferred with at least 2 years of DSP experience.
- Prior supervisory experience required.
- Ability to establish and maintain a working relationship with program participants, families, staff and community agencies.
- Ability to communicate both orally and in written form.
- Knowledge of Person Centered Planning.
