Job Description :

This position reports to the Clinical Supervisor and is responsible for providing milieu-based therapy for patients primarily ages 5 to 13 and enrolled in grades 1st through 8th at Mount Saint Vincentâ€s Day Treatment Program.

The School Social Worker is responsible for directing all aspects of the patient and familyâ€s clinical care within the educational setting, and works in close partnership with the Teachers, Educational Mental Health Workers and school staff.

As a School Social Worker you need to know how to :

  • Provide consistent affective education and therapy to patients within the school, and their families. Therapy includes milieu-based individual therapy, family therapy, and group therapy
  • Utilize a Social Emotional Learning (SEL) platform within the school. Responsible for implementing and overseeing assigned SEL courses to patients on your caseload
  • Actively engage with patients in the classroom milieu according to the daily schedule and individual treatment plans
  • Conduct assessments including comprehensive diagnostic evaluations and Functional Behavior Assessments (FBA)
  • Write reports including initial evaluations / assessments, treatment plans, treatment notes, monthly updates, discharge plans, as well as all other required documentation
  • Monitor caseload for treatment progress or regression and update treatment plans as necessary in compliance with agency timelines
  • Collaborate and consult with appropriate stakeholders
  • Provide case management for the patients and families on your caseload
  • Responsible for discharge planning, providing aftercare recommendations, and providing aftercare referrals as appropriate
  • Attend, actively contribute to, and / or facilitate required meetings, which includes Individualized Education Plan (IEP) meetings for patients on your caseload
  • Maintain compliance with agency policies / guidelines and regulatory standards
  • Is sensitive to the cultural differences among the children, families and staff and responds appropriately and sensitively to those differences.

Preferred :

  • State licensure as Licensed Clinical Social Worker (LCSW) strongly preferred
  • One (1) year relevant experience preferred

Physical Requirements :

  • Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.-
  • Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
  • Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.

  • LSW or CSWC, or the ability to obtain LSW or CSWC within three (3) months of hire
  • Masterâ€s Degree in Social Work (MSW)
  • Training in child development, psychotherapy, family dynamics, systems and cultural differences
  • Current CDE Special Service Provider License or the ability to obtain within six (6) months of hire
  • Current Driverâ€s License and must obtain Colorado Driver's License within 30 days of moving to Colorado
  • Motor vehicle record and proof of automobile insurance coverage to be updated annually
