Day Kimball Healthcare is hiring for an Advanced Practice Registered Nurse for the Full Time Day Shift Internal Medicine job in Plainfield, CT.

As the Advanced Practice Registered Nurse (APRN), you will be delivering patient care using a collaborative health care model including physicians, other advanced practice nurses, nursing staff, medical office clinical staff, patients, families, and other supportive health care providers working together to provide a holistic approach to quality patient care.

The NP will exercise clinical leadership through clinical practice, consultation, staff, patient and family education, research, and professional development.

The Advanced Practice Registered Nurse will be working in the inpatient and outpatient settings, assess, manages, prescribes, and evaluates appropriate therapeutic and pharmacological interventions for patients with common illnesses and conditions in the cardiology field, according to standards of practice.

Consults or makes referral to primary and specialty care physicians, other health care professionals, and other services, facilities or agencies when a situation is beyond their level of expertise or when additional services are needed to coordinate patients’ medical care.

Advanced Practice Registered Nurse Job Responsibilities :

  • Possesses ability to assess data reflective of the patients' status and interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs.
  • Establishes a diagnosis, develops a plan of care in collaboration with patient and their family and evaluates clinical outcomes through follow-up visits and / or telephone contact.
  • Orders, obtains and interprets laboratory and diagnostic tests when indicated according to standards of practice and / or in collaboration of physicians.
  • Documents finding of physical and developmental assessment and plan of care in a systematic manner.
  • Consults / collaborates with interdisciplinary team members regarding assessment and implementation of the plan of care and evaluation of outcomes as indicated.
  • Makes appropriate referrals to members of the health care team to optimize patient / family care.
  • Cooperates with other professionals and agencies involved in providing services when appropriate, coordinates the health care given to promote continuity of care.
  • Evaluates efficacy of care in achieving optimum health state, cost effectiveness and quality patient care.
  • Conducts preventative screening as needed.
  • Identifies and initiates appropriate non-pharmacologic interventions.
  • Obtains a comprehensive health history.

Advanced Practice Registered Nurse Education and Experience Requirements :

  • If licensed after 1999 must have Master's Degree in Nursing from an accredited Nurse Practitioner program required
  • Must have a nationally recognized certification to practice.
  • Current Federal DEA and State Controlled Substance License required
  • Current Connecticut State RN / APRN Licensure
  • 5-7 Years experience in clinical setting providing direct patient care in the field of Cardiology required

Advanced Practice Registered Nurse Skills and Abilities :

  • Ability to discriminate between normal and abnormal findings in the history and physical exam in order to establish a diagnosis.
  • Ability to assess severity of patients' illness and establish need / time for patient's evaluation.
  • Ability to plan and care for patients identified problems including medication, diet, therapeutic procedures, etc.
  • Ability to consult with other health team members regarding proposed treatment guidelines.
  • Ability to form therapeutic relationships with patients.
  • Demonstrate appropriate communication, interpersonal and organizational skills; and ability to maintain positive relationships with physicians, nurses and other hospital personnel.
  • Demonstrate leadership and advanced clinical decision-making skills.
  • Demonstrate cultural sensitivity to diverse populations.
  • Demonstrates responsibility for continual professional development and commitment to advanced practice nursing.
  • Must be self-directed, creative and able to work independently.
