Registered Nurse - RN - Case Manager

• Schedule : 4 10 hour shifts\n
• Work Location : UAB St Vincent's Birmingham\n
• Benefits for eligible positions only include : generous paid time off, paid parental leave, Associate Assistance Program, Tuition Reimbursement Program, and more\n\nWhat You Will Do\n\nCoordinates the overall interdisciplinary plan of care for patient, from admission to discharge.\n
• Creates plan for care across the continuum, integrating patient / family preferences and values.\n
• Monitors patient medical necessity and level of care through assessments, ongoing evaluations and / or patient records.\n
• Advocates for resources and removal of barriers.\n
• Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan.\n
• Acts as a resource for adequate medical record documentation, level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care.\n\nWhat You Will Need\n\nLicenses / Certifications / Registrations\n
• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.\n
• Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing required.\n\nEducation : \n
