UC Health is hiring a LPN for SNF 2North, for a full-time at Daniel Drake Center for Post-Acute Care.
The LPN will support Daniel Drake's SNF 2 North department, delivering high-quality care to patients based on assessed needs, established standards of care, and according to policy and procedures.

Daniel Drake Center specializes in providing post-acute and transitional care services for patients recovering from a catastrophic injury or illness.
Daniel Drake Center is the only hospital-based skilled nursing facility in the Cincinnati region. Job Responsibilities
The LPN is an individual who holds a current state license to practice nursing. The practice of nursing requires specialized knowledge, judgment, and skills to provide care to groups and individuals.
The LPN utilizes knowledge derived from the principles of biological, physical, behavioral, social and nursing sciences to assess, plan, implement and evaluate patient care.
The care is culturally based and age-specific.
- Contributes recommendations to the RN regarding the interdisciplinary plan of care for each assigned patient in a collaborative effort with other health care team members within the time frame indicated by unit's standards of practice.
- Provides input to the RN regarding establishing and prioritizing a list of actual, and potential, individualized age-specific patient problems and / or needs.
- Communicates formulated plan of care, as appropriate, to all levels of associates involved in the patient's care and demonstrates ability to establish priorities of care based on patient's assessment and needs.
- Gives clear instructions to patients / family regarding treatment, and involves family / guardian in the assessment, initial treatment, and continuing care of the patient.
- Identifies and responds appropriately to different patient needs resulting from behavioral health problems, unique psychological needs or those associated with religious / cultural norms.
- Demonstrates critical thinking, accountability, flexibility, and a cooperative attitude in meeting the needs of the unit.
- Nursing process is accurately and concisely documented, including the patient's response to nursing interventions and in accordance to hospital policy and procedure.
- Acts as a resource person and mentor for new personnel, floater, and / or supplemental staff working on the unit.
- Assumes responsibility for personal professional development and contributes to the professional development of peers, colleagues, and others.
Qualifications
Education and Experience Requirements :
- Graduate of an accredited school of practical nursing.
- Current Ohio Board of Nursing - IV Certification

- Effectively communicates with peers, utilizes appropriate communication channels and maintains confidentiality.
