Position Summary : Provides evaluations and treatments to patients in an outpatient setting to improve or restore function.
Collaborates with interdisciplinary team and participates in clinical education of students. May be responsible for cross-site coverage.
Education / Certification :
- Graduate of an accredited program of Physical Therapy with a minimum of a B.S.
- Must hold a current state license in Physical Therapy.
Essential Duties and Responsibilities :
- Evaluates patients based on departmental standards
- Develops individualized treatment plans based on patient's symptoms and goals
- Administers therapeutic exercises and physical agents
- Provides patient education for home exercise programs and functional activities
- Documents patient progress and adheres to department's policies
- Participates in interdepartmental communication and professional development
