JobDescription andResponsibilities :
We are in search of a great Director of Case Management to run anoffice with a great team. This role requires a candidate to have atleast 35 years of progressive leadership.
- Responsibleand accountable for the management of staff and services related toCase Management.
- Accountable forthe effective and efficient management of continuum of careservices.
- Provides education andleadership in directing a comprehensive Clinical Case Managementprogram that coordinates efforts toward a seamless Continuum ofCare.
- Assists leadership to ensurethe stability and reputation of the hospital by ensuring compliancewith all legal licensure and accreditation requirements.
- Acts as a liaison to the CentralBusiness Office to ensure that all Accounts and Revenue activity isbeing handled timely and appropriately.
- Maintains 24hour accountability.
- Embrace and consistentlydemonstrate our hospital initiatives encompassing both patients andemployees.
Requirements
Knowledge / Skills :
- Demonstratescomprehensive knowledge of Case Management in the Acute Caresetting.
- Comprehension of theimportance of clinical care progression patient education andresource management required.
- Knowledge of Utilization ReviewIS / SI criteria reimbursement issues and HCFA guidelines.
- Ability to recognize the needs andconcerns of people and to maintain constructive relationships indealing with them.
- Ability tocommunicate effectively orally and in writing.
- Participates in counseling andguidance of personnel.
- Acts as amediator between patient concerns and administration.
- Ability to assume a leadership rolewith the Case Management Team to coordinate the delivery ofservices for patients and theirfamilies.

Current BLS / ACLS / PALS issued by the American Heart Associationrequired. UM / CM certification preferred. We need someone who hasworked in acute care in the past 5 years and would prefer level 1trauma experience.
Minimum of 3 years recent clinical or CaseManagement experience required. Previous management experienceREQUIRED.
