Description :
This person will work 3 days per week on site supporting both the kidney and liver team with one work from homeday if needed.
Must work at least 3 days per week for work from home privileges to apply.
JOB PURPOSE : Collaborates with medical provider and clinical team to provide patient / family advocacy, assessment, discharge planning coordination, and psychosocial interventions for high-risk populations both inpatient and outpatient.
The Social Worker strives to promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources, and manages, supports and develops comprehensive transitional care plans for patients with complex health care needs in Inpatient, ED and Outpatient environments.
DUTIES WILL INCLUDE THE FOLLOWING :
- Complete psychosocial evaluations for transplant candidates and living donors.
- Communicate with patients via phone to assist with transplant listing and post-transplant needs.
- Provide coverage for absences.
- Assist both kidney and liver teams with managing caseloads for pre, waitlist and post.
- Assist with outreach as needed.
Qualifications :
MINIMUM EDUCATION REQUIRED :
Masters degree in Social Work (LMSW)
MINIMUM EXPERIENCE REQUIRED :
Two years of medical experience (hospital, nursing home, home health care, or dialysis)
Knowledge of Social Work processes with skills in counseling, case work, group work and community services.
Working knowledge of community resources and financial assistance programs.
MINIMUM LICENSURE / CERTIFICATION REQUIRED BY LAW :
LMSW or LCSW in the State of Georgia
ADDITIONAL PREFERRED QUALIFICATIONS :
Previous transplant experience or experience working with patients with end-stage organ disease preferred.
