LICENSED VOCATIONAL NURSE/LICENSED PRACTICAL NURSE

Credentials :

• Currently licensed as a Licensed Practical Nurse or Registered Nurse in the State of Georgia
• Graduate of an accredited practical nurse or vocational nursing program
• One year of prior professional nursing experience, homecare experience preferred
• Tracheostomy and ventilator experience preferred
• Current CPR certification
• Clean background check

Job responsibilities :

• Changes in the patient’s condition are identified and documented with resolution on the appropriate clinical form
• Physician notification to obtain necessary orders for intervention(s) per state regulations
• Performs specific treatments and medication administration in accordance with physician orders
• Documentation reflects patient / family collaboration and participation in their plan of care
• Documentation and clocking in and out utilizing the current patient health record platform, Alayacare
• Skilled observations and significant changes in patient status / care are communicated to the Director of Clinical Services and / or Clinical Supervisor / Case Manager
• Request for modification of orders and additional orders are made based on the patient’s clinical status and Skilled nursing judgment
• All physician collaboration / coordination of care is documented in the medical record and communicated as needed
• Adheres to H-squared Homecare policies and procedures
