2.1.Staff / Facility
2.1.1. License : The Contractorsphysician(s) assigned by the Contractor to perform the servicescovered by this contract shall have a current license to practicemedicine in any State Territory or Commonwealth of the UnitedStates or the District of Columbia when services are performedonsite on VA property.
All licenses held by thekey personnel working on this contract shall be full andunrestricted licenses. Contractors physician(s) who have currentfull and unrestricted licenses in one or more states but who haveor ever had a license restricted suspended revoked voluntarilyrevoked voluntarily surrendered pending action or denied uponapplication will not be considered for the purposes of thiscontract.
2.1.2. Board Certification : Contractors physician(s) shall be Board Certified / Board Eligibleby the American Board of Anesthesiology (ABA) and be currentlycertified in Basic Life Support (BLS) Advanced Cardiac Life Support(ACLS) or equivalency.
All continuing education courses requiredfor maintaining certification must be kept up to date at all times.Documentation verifying current certification shall be provided bythe Contractor to the VA COR on an annual basis for each year ofcontract performance.
2.1.3. Credentialing andPrivileging : Credentialing and privileging is to be done inaccordance with the provisions of VHA Directive 1100.
20 and VHADirective 1100.21 referenced above. The Contractor is responsibleto ensure that proposed physician(s) possesses the requisitecredentials enabling the granting of privileges.
No services shallbe provided by any Contractors physician(s) prior to obtainingapproval by the Facility Medical Executive Board and Medical CenterDirector.
2.1.3.1. If a Contractors physician(s)and / or other contract provider(s) are not credentialed andprivileged or has credentials / privileges suspended or revoked theContractor shall furnish an acceptable substitute without anyadditional cost to the government.
2.1.4.Technical Proficiency : Contractors physician(s) shall betechnically proficient in the skills necessary to fulfill thegovernments requirements including the ability to speak understandread and write English fluently.
Contractor shall provide documentsupon request of the CO / COR to verify current and ongoing competencyskills certification and / or licensure related to the provision ofcare treatment and / or services performed.
Contractor shall provideverifiable evidence of all educational and training experiencesincluding any gaps in educational history for all Contractorsphysician(s) and Contractors physician(s) shall be responsible forabiding by the Facilitys Medical Staff ByLaws rules and regulations(referenced herein) that govern medical staff behavior.
2.1.5. Continuing Medical Education (CME) / CertifiedEducation Unit (CEU) Requirements : Contractor shall provide the CORcopies of current CMEs as required or requested by the VA facility.
Contractors physician(s) registered or certified bynational / medical associations shall continue to meet the minimumstandards for CME to remain current.
Contractor shall report CMEhours to the credentialing office for tracking. These documents arerequired for both privileging and reprivileging.
Failure to provideshall result in loss of privileges for Contractorsphysician(s).
2.1.6. Training (ACLS BLS EHR andVA MANDATORY) : Contractor shall meet all VA educationalrequirements and mandatory course requirements defined herein;
