Provides appropriate and clinically indicated case management services to children, adolescents and adult consumers and their families.

Documents all services provided within the clinical record. Engages in extensive marketing and outreach activities to individuals, organizations and communities.

PRIMARY JOB RESPONSIBILITIES

Provides outreach, coordination, advocacy, and linkage services for child, adolescent and adult consumers and their families to ensure clinical continuity.

Reaches out to difficult-to-engage consumers, makes courtesy calls and outreach attempts to remind consumers of scheduled clinic appointments, and follows up with consumers who have missed a scheduled appointment.

  • Assists consumers in meeting concrete service needs. Makes referrals and linkages to appropriate agencies including, but not limited to, housing, educational / vocational training, and entitlement services.
  • Accompanies and escorts consumers to community appointments including, but not limited to medical, educational / vocational, housing and entitlements.
  • Identifies opportunities for and provides community outreach regarding services offered by ICL and its Article 31 clinics, and provides community presentations on a variety of informational topics.
  • Collaborates with an interdisciplinary team.
  • Participates in service planning, case conferencing, and goal reviews.
  • Completes all necessary documentation of daily services, including daily progress notes, collecting and compiling daily and monthly statistics, and any necessary referral / linkage material.
  • Participates in agency training / in-service programs.
  • Attends weekly individual supervision with clinical supervisor.

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES :

  • Ability to work with clients / residents, families, and staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences.
  • Ability to serve as a role model to residents / clients.
  • Demonstrated clinical competence.
  • Knowledge of both verbal and written English at the Associate College level and ability to follow written and oral instructions.
  • Ability to react appropriately in emergency situations.
  • Ability to perform and interact as part of a multi-disciplinary team.
  • Basic working knowledge of computers, and ability to complete written forms and reports in an accurate and timely manner, manually and by computer.
  • Ability to communicate effectively with staff, clients / residents, families, and the public.
  • Ability to accompany or transport residents.
  • Ability to secure and maintain certification for Standard First Aid and CPR.

JOB SPECIFICATIONS :

Education : High School Diploma required. Associate’s or Bachelor’s degree in an allied health field from an accredited university, preferred.

Training and Experience : Minimum two years social services experience required.

Job Knowledge : Must have good written and verbal communication skills. Cultural competency essential.
