PRIMARY CARE NURSE PRACTITIONER

Salary : $145,831.32 - $169,274.69 per year
Working closely with Physicians and the IDT, the Nurse Practitioner undertakes assessments, develops care plans, delivers primary care, and provides continuing medical education to participants as directed by the Medical Director.
Key Responsibilities :
- Conduct comprehensive in-person assessments including initial, semi-annual, and unscheduled assessments, while offering continuous medical coverage to participants
- Collaborate with physicians and IDT to devise individual care plans for participants
- Support physicians and IDT in the discharge planning process from acute or long-term care settings
- Ensure ongoing care for participants in nursing homes, conducting regular visits based on participant needs
- Work in collaboration with physicians, community-based PACE physicians, and IDT to oversee and ensure effective care coordination
- Review outcomes and utilization data including hospitalizations and specialist usage, under the guidance of the Medical Director, to establish best practices and policy enhancements
- Participate in rotational night and weekend call schedule
Job Requirements :
- Completion of an Accredited School of Nursing program
- Nurse Practitioner certification with Prescriptive Authority
- Active RN State License
- Minimum of two years' experience working with the senior population
- Up-to-date knowledge of chronic care / geriatric issues and best practices
- Valid State Driver’s License with a clean DMV record
- Submission of recent Vehicle Insurance documentation

SKILLS AND CERTIFICATIONS
- Certified Nurse Practitioner with Prescriptive Authority
- Active California license
- Experience in Geriatrics (preferred)
- 2 years of Nurse Practitioner experience
