Job Title : Rehabilitation Therapist Job Specialty : Physical Therapist Job Duration : 13 Weeks Shift : Monday - Friday Guaranteed Hours : 32 Hours Experience : New Graduates Accepted, SNF Experience Preferred License : State License Required Certifications : COVID Test Required 72 Hours Prior to Start Date (Results Issued 48 Hours Prior) Must-Have : - State License Verification- Updated Resume- Three References- County Criminal Check (7 years for all counties lived and worked)- Federal Criminal Check (7 years or more)- Multi-State Criminal Search (7 years or more)- Nationwide Sex Offender Search (7 years or more)- HHS / OIG Search- GSA Search- Social Security Number Trace- 2-Step PPD or Quantiferon or Proof of Positive PPD and Chest X-Ray- SAM Search- Oregon State Background - CRIMS (facility run) Job Description : Ensure prescreen is filled out and attached upon submitting a candidate.
- Attach resume and license verification for submission.- Must pass a credentialing checklist prior to hiring.- Participate in therapy sessions and create a supportive environment for patient rehabilitation.
- Collaborate with other healthcare professionals to develop treatment plans.- Maintain detailed and accurate patient records.
- Adhere to all facility policies and procedures.- Conduct appropriate patient assessments and modify treatments as necessary.
- Promote patient education and involvement in their own recovery process.
