Day Treatment Case Manager - Full-Time

Easterseals of Mahoning, Trumbull and Columbiana Counties is looking for energetic and compassionate Day Treatment Case Managers to serve our clients and help to make a difference.

The position is Full-Time with 9-month stretch pay and off during summer.

Basic Responsibilities :

JOB OBJECTIVE :

This position is primarily located within a specialized classroom for students with emotional disturbance or multiple disabilities.

Primary responsibilities include providing intensive, structured, goal-oriented mental health day treatment services.

ESSENTIAL DUTIES :

• Provide intense treatment service that consists of high levels of face-to-face mental health interventions that address the individualized mental health needs of the client as identified in their individualized service plan (ISP).
• Conduct groups and activities that address skills development such as functional relationships with adults, peers, and community, family, and authority figures.
• Assist with the development of problem-solving skills, conflict resolution, emotions / behavior management, and developing positive coping mechanisms.
• Manage mental health and behavioral symptoms to enhance vocational / school opportunities and / or independent living.
• Provide psycho-educational interventions including individualized client instruction to increase knowledge and understanding of psychiatric treatment and to increase their cooperation and collaboration with treatment and rehabilitation.
• Assist the individual with effectively responding to or avoiding identified precursors or triggers that would risk their remaining in a school setting or that result in functional impairments, including assisting the individual, teachers, and family members with identifying a potential psychiatric crisis, developing a crisis management plan, and / or, as appropriate, seeking other supports to restore stability and functioning.
• Utilize Trauma Informed practices when appropriate.
• Attend and participate in staff meetings, in-service training, and supervision as directed by the Clinical Supervisor.
• Follow agency guidelines on documentation and productivity standards.
• Maintain productive and professional relationships with school personnel and other agency staff.
• Work effectively within a school setting following their rules and regulations and following the lead of the of school administrator when necessary.
• Collect outcomes-related data as well as grant data if applicable.
• Attend all training required such as CPI, SEL, trauma-informed care.
• Maintain current working knowledge of all FCS policies, school policies, COA standards, and OMHAS standards.
• Job Assignment is contingent upon the direction of the partnering school district / ESC and can be severed at any time based on the expressed wishes of the district / ESC.

Continued employment will be at the sole discretion of the employer.

• Perform other assigned and related duties specific and customary to the department in which they are assigned.

Qualifications :

• Bachelor’s Degree in social work or psychology or related field. Masters Degree preferred.
• Experience working in the behavioral health field is preferred.
• The skills and knowledge in the areas of behavioral modification, and behavioral health issues.
• The ability to respond appropriately to crisis or emergency situations.
• The ability to communicate effectively verbally and in writing.
• The ability to conduct mental health groups.
• The ability to establish and maintain effective relationships with clients, their families, educational personnel, and agency staff.
• Must be able to meet agency requirements for criminal background checks (Ohio Bureau of

Criminal Identification & Investigation).

EQUIPMENT USED / MACHINERY REQUIRED TO OPERATE :

General office equipment including computers and applicable software programs.

PHYSICAL DEMANDS :

• Stands, walks, bends, squats, twists, and kneels frequently.
• Reaches overhead on occasion
• Pushes / pulls up to 25 pounds occasionally.
