The Home Health Registered Nurse is responsible forproviding and documenting skilled one-on-one nursing care in accordance withthe physician-ordered individualized patient care plan.
This care is provided mostly in the patient’shome or a residential setting but can transfer outside the home duringappointments and other activities the nurse may accompany the patient to.
Essential Duties and Responsibilities : Completes an initial assessment of patient and family todetermine home care needs when applicableProvides a complete physical assessment and history ofcurrent and previous illness(s)Regularly re-evaluates patient nursing needsInitiates the plan of care when directed and makes necessaryrevisions as patient status changesUses health assessment data to determine nursing diagnosisEstablishes goals based on nursing diagnosis andincorporates therapeutic, preventative, and rehabilitative nursing actionsIncludes the patient and the family in the planning andexecution of the treatment plan and goalsInitiates appropriate preventative and rehabilitativenursing proceduresAdministers medications and treatments as prescribed by thephysicianProvides healthcare instructions and education to thepatient as appropriate per assessment and plan of careIdentifies discharge planning needs when applicableAdheres to home health agency policy and protocolAttends case conference, team meetings, and educationin-services as requiredMinimum Requirements : Graduate of an accredited school of nursingCurrently licensed as a Registered Nurse in the state inwhich the RN will practiceComplies with all relevant professional standards ofpracticeParticipation and completion of Amergis' Competency programwhen applicableCurrent CPR if applicableTB questionnaire, PPD or chest x-ray if applicableCurrent Health certificate (per contract or stateregulation)Must meet all federal, state and local requirementsSuccessful completion of new hire training as applicable tojob siteUnderstand patient confidentiality and HIPAArequirementsAbility to effectively elicit / provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills;
