Medical :
- 80% of monthly medical premium costs covered for single plans, 75% covered for individual plans
- CHCB covers 80% of the Deductible costs for a single plan and 75% of the deductible costs for a family plan through a Benefit Debit Card.
Dental :
Preventive dental services covered 100%, basic dental services covered 80% and major dental services covered at 50%.
Vision :
Yearly eye exam $10 deductible, lenses covered in full, $150 frame allowance. Frames or lenses subject to $25
deductible. Contact lenses reimbursed $150 if elective, if medically necessary, covered in full.

QUALIFICATIONS :
- Seeking candidates who demonstrate a passion for outreach and community support. The ideal candidate will identify and empathize with our patient population and should understand the nuances of being a not-for-profit.
- Accepting BC Candidates only, that have completed 3600 hours and have independent practice authority within the state of NY
