Licensed Mental Health Therapist
JobDescription :
providing best in classmental health services that will be delivered via telehealth and inperson designed specifically to treat addiction and mental healthconditions for the populations we serve.
Job Duties :
- Facilitates traditional outpatient and intensiveoutpatient programs and services for Mental Health conditions andsubstance use disorder including individual and family therapy aswell as case management for a caseload of individual clientservices.
- Responsible for assessing client forcooccurring and mental health needs using DSM criteria.
- Completion of Initial Assessments for clients as well asassessing for appropriate level of care.
- Obtain and review records relevant to the currenttreatment episode and contact to relevant referral sources familyor outside systems as needed.
- Prepare (asneeded) and maintain treatment plan and necessary revisions inconjunction with the case manager.
- Coordinatewith discharge planner regarding continuing care andrecommendations for discharge.
- Documentationof all counseling services in clinical records.
- Maintain all necessary logs required for administrativetracking.
- Complete relevant staterequired workfor clients on your caseload.
- Participate as amember of a multidisciplinary team assigned to the client andparticipate in case management meetings.
- Conduct individual and group counseling.
