Responsibilities and job duties :

As an RN perform the initial homecare visit and re-evaluate the patient’s needs and progress on a regular basis. Initiate the plan of care under doctor’s orders.

Perform admission, transfer, re-certification, resumption of care, and discharge Oasis for the home care patient. Perform medication reconciliation.

• RN observe and assess patients, and document patient assessments. Monitor reactions and patient progress to treatment.
• Educate patients and caregivers on disease process, medications, plan of care, and individualized treatment plans.
• Educate patients and caregiver on techniques for in home health care.
• Communicate and coordinate with other members of the healthcare team.
• RN to supervise LPNs and HHA’s and provide oversight of the care plan. Provide onsite Supervision of LPN & HHA.
• Notify the physician and other personnel (Clinical Manager, Therapist, and Case Manager) of change in the patient’s condition.
• Perform skill outline in the agency’s approved policy and procedure manual. Discharge the patient from skilled nursing services when the discharge criteria have been met.
• Case conference clinicians providing care to ensure coordination of care.
• Update clinical records according to policy and procedures.
• Update knowledge and skills by attending in-service programs, continuing education programs, seminars, and self-study programs annually.
• Adhere to Medicaid and Medicare regulations.

Skills

• Proven experience as a registered nurse
• Excellent knowledge of nursing care methods and procedures
• Excellent knowledge of home healthcare
• In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times
• A team player with excellent communication and interpersonal skills
• Responsible and compassionate
• Outstanding organizational and multi-tasking skills
• Patient with excellent problem-solving skills

Qualifications :

• Nursing : 2 years (Required)
• Valid and Current Missouri RN license

Offering both full and part time positions.

Shift lengths anywhere from 4hrs-12hrs

Start times are flexible.

Part-time 6- 31hours each week

Full-time 32- 40hrs each week
