New leadership : This Skilled Nursing Facility (SNF) is owned and operated by Stephenson County, but now managed by Transitional Care Management .
We have a new Administrator, new clinical programming and we are expanding our team.
With 148 beds, we serve a diverse population of short-term rehab patients, long term care residents and memory care. We have an active therapy dept.

Job Description
This is a Floor Nurs position. An overview of the expecations are :
- No benefits / insurance offered with this position
- This is a nonunion position
- This is an as-needed , intermittent schedule.
- You must commit to working a 2 weekend shifts per pay period. You can also pick up additional shifts as your schedule allows.
- We require our PRN staff to attend a full New Employee Orientation.
Qualifications
- RN or LPN license in IL
- Prior experience in a skilled nursing facility
- Experience with the short term rehab population is helpful
- Electronic medical records experience (Matris) is ideal
