Job Title : On-Call Hospice RN – 7 Days On / 7 Days Off (Full-Time)

Location : Greater Twin Cities Metro Area – 21 County Service Area

Compensation : $90,000–$130,000 / year, depending on experience

Schedule : On-call for 7 consecutive days , followed by 7 days off

Your Key Responsibilities :

  • Serve as the primary clinical contact for hospice patients after hours and during weekends
  • Perform in-home visits as needed to assess pain, symptom changes, or end-of-life care needs
  • Provide empathetic telephonic support to families and caregivers
  • Coordinate with the interdisciplinary group (IDG) to ensure continuity and quality of care
  • Document patient encounters and updates accurately in our EMR system
  • Remain on-call continuously for all 7 active days.
  • What We’re Looking For :

  • Active RN license in Minnesota
  • Minimum 2 years of hospice, oncology, or palliative care experience
  • Ability to remain calm under pressure and make sound clinical decisions independently
  • Reliable personal vehicle and willingness to drive within a 21-county radius
  • Must use your personal smartphone for clinical calls and documentation (reimbursed monthly)
  • Commitment to maintaining a patient-first, high-integrity approach to care
