Shift Type / Shift Times / Hours / Day : Nights, 7 : 00 PM - 7 : 30 AM, 12

Shift Notes : 7 : 00 PM - 7 : 30 AM / 36 hours scheduled / 24 hours guaranteed / the client is allowed to cancel 1 shift per week / Must be willing to float to units where experienced

Weekend Requirements : Weekend schedule to be determined by facility

On-Call Requirements : On-call schedule to be determined by facility

COVID-19 / FLU Requirement : Flu vaccination - REQUIRED EFFECTIVE 9 / 16 Please add vaccination status in the submission notes

Traveler Policy : No candidates with an AZ home address on their driver's license will be considered - please include a copy in the submission

UPDATED ASSIGNMENT TERMS FOR ALL STARTS ON OR AFTER 4 / 23 / 2023

• Learning modules must be completed before the start of the assignment.
• Non-billable hours have been adjusted to reflect 16 hours for each assignment.
• Client or Vendor may cancel an Assignment in progress with 7 days written notice without penalty.

The client will not approve RTO within the first two weeks or holidays. Can not exceed 7 days.

This helps us get candidates presented quicker as we all know these orders open / close quickly!

This form needs to be 100% accurate and have all requests noted in the highlights section.

• Start Date should be on or very quickly after the Order Start Date listed.
• All RTO should be listed on the cover page and must be less than 7 days.
• RTO will not be approved in the first 2 weeks of the assignment.
• All facilities the traveler is open to working in should be listed on the submission.
• If interested in "any" location, please include what states (I.E. Any AZ, Any CO, Any state, etc.)
• All shifts the traveler is open to working on should be listed on the submission.
• All departments the traveler is open to working must be checked on the cover page.
• All candidates submitted should be preclosed and ready to accept.
• Please let us know if the traveler withdraws or is no longer available.

Submission Requirements

• Minimum 24 months of current PCU experience - REQUIRED
• Certifications must be American Heart Association OR American Red Cross and active upon submission - MUST BE INCLUDED IN SUBMISSION - REQUIRED
• No Nursys Hits - REQUIRED
• Travel experience - Preferred
