Home Visit Behavioral Health Specialist - Florida

Seeking a Behavioral Health Specialist in Florida with expertise in at least one of the following areas : behavioral / mental health, experience with SMI population, healthcare, home health and / or Medicaid.

We encourage individuals to apply that have experience as Behavioral Health Technicians, Case Managers, Community Health Workers, Home Health Aide, Certified Nurse Assistants, Medical Assistants, etc.

Location : This is a field-based position (at least 50% of the time). Seeking candidates that reside in the state of Florida and in the following counties : Indian River, Martin, Okeechobee, Palm Beach, St.

Lucie, Broward, Miami-Dade and Monroe.

Work Schedule : Monday - Friday, Flexibility to work between 8am - 7pm EST and weekend availability as needed.

The Home Visit Behavioral Health Specialist is responsible for providing support with finding resources for our behavioral health members in the comfort of their homes and / or in their communities.

Also, ensuring that appropriate member treatment plans are followed on less complex cases and for proactively identifying ways to improve the health of our members and meet quality goals.

How you will make an impact :

  • Coordinates follow-up care plan needs for members by scheduling appointments or enrolling members in programs.
  • Conduct behavioral health intakes and assessments to understand members needs and social determinants of health.
  • Assesses member compliance with medical treatment plans via telephone or through on-site visits.
  • Identifies barriers to plan compliance and coordinates resolutions.
  • Identifies opportunities that impact quality goals and recommends process improvements.
  • Recommends treatment plan modifications and determines need for additional services, in conjunction with case management and provider.
  • Coordinates identification of and referral to local, state or federally funded programs.
  • Coaches members on ways to reduce health risks.
  • Prepares reports to document case and compliance updates.
  • Establishes and maintains relationships with agencies identified in appropriate contract.

Minimum Requirements :

Requires a high school diploma and a minimum of 1 year related experience; or any combination of education and experience which would provide an equivalent background.

Preferred Skills, Capabilities & Experiences :

  • Experience with complex psychiatric issues and medical disorders is highly preferred.
  • Experience in health coaching and motivational interviewing techniques.
  • Knowledge of Social Determinants of Health (SDOH) barriers and resources.
  • Comfortable working with SMI members in their homes and / or in the community.
  • Certified nurse assistant (CNA) or certified medical assistant (MA).
  • BS / BA degree in a related field preferred.
  • Bilingual communication skills in English and Spanish preferred.
  • For URAC accredited areas, the following professional competencies apply : Associates in this role are expected to have strong oral, written and interpersonal communication skills, problem-solving skills, facilitation skills, and analytical skills.
