Purpose :
Provides services across the Maternal, Reproductive, and Child Health Service line in the community to at-risk individuals.
Responsibilities :
- Provides strength-based, family-centered, comprehensive care that assists clients in reaching an optimal state of health in a variety of community settings, including but not limited to homes, the judicial system, and telephonically.
- Assesses the assigned population based on the needs of the contracted services and creates plans for continued health treatment or makes referrals based on available resources.
- Provides nursing case management and care coordination to clients and families, may include home visit caseload.
- Provides services, immunizations or testing to assigned population of clients, in accordance with best practice standards and terms on program contract.
- Maintains accurate and timely documentation in clinical and billing records and databases.
- Teaches and advises clients and families on a variety of health-related issues such as the prevention of disease and disability, TB screening and testing, reproductive life plans, maternal health, and child health.
- Assists participants in establishing and maintaining medical / dental homes to obtain comprehensive health care.
- Informs physicians and other personnel of changes in the client's condition and nursing needs.
- Works to maintain a safe environment for the client and / or infant.
- Represents EveryStep in a positive and professional manner, communicates effectively and respectfully with team, manager and community partners, and maintains confidentiality at all levels of the agency.
- Complies with accepted professional standards and ethical guidelines.
- Confers with other agencies regarding client needs; exchanges information and attempts to determine a solution for such needs.
- Manages time, organization, and scheduling to meet client needs as well as other job duties and responsibilities.
- Utilizes language interpretation / translation services and obtains pertinent information according to the culture and primary language of each individual as appropriate.
- Engages in on-going personal professional development and training.
- Cross-trains in other Community Health programs to create a well-rounded team of providers to administer treatment and services based on specific requirements.
- Performs other duties as assigned by supervisor.
Qualifications :
Current and valid Iowa RN licenseBachelor's degree in nursing from an accredited school of nursing preferredMinimum of two years of clinical experience in maternal child health nursing or related fieldKnowledge of professional nursing principles and practices, including knowledge of family-centered nursing careWorking knowledge of maternal child health nursing principles and practices, including community health problems, preventive measures, and community resourcesAbility to plan, organize, and utilize teaching principles in working with families, clients, schools, and community organizationsAbility to exercise independent nursing judgmentExcellent critical thinking and decision-making skillsAbility to accept and work with diverse populationsAbility to communicate effectively, both verbally and in writing, using the English languageAbility to physically carry out the various aspects of providing services in the client's home, school, or other environmentsCurrent American Heart Association Healthcare Provider certificationMandatory Reporter of Child and Dependent Adult abuse training certification (within 30 days of hire)Must have valid driver's license, auto liability insurance and reliable transportation if driving is requiredSuccessful completion of pre-employment physical, drug screen, 2-step TB test and background checkWorking Conditions :
