About The Position
Job Title : Physical Therapist - Home Health Job Specialty : Physical Therapist - Home Health Job Duration : 13 Weeks Shift : Day - 8x5 - 8 AM - 4 PM - 40 Hours Guaranteed Hours : Not Specified Experience : Minimum of 1 year of home care experience required License : Physical Therapist Professional License in Massachusetts Certifications : Not Specified Must-Have : - Ability to travel to assigned cases- 1 year of home care experience- Weekend rotations once every 6 weeks Job Description : Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes- Collaborate with patients, their families, physicians, and other allied health professionals to identify patient needs and coordinate services to meet those needs- Utilize thorough and timely electronic documentation- Engage in interdisciplinary communications following agency policies, procedures, and standards of practice- Work with a diverse age range of patients, from birth to over 100 years
