Central City Concern is an innovative nonprofit agency providing comprehensive services to single adults and families in the Portland metro area who are impacted by homelessness, poverty and addictions.

Location : The Starlight : 355 NW 6TH AVE., PORTLAND,OR

Schedule : Monday - Friday 9am - 5 : 30pm

SUMMARY :

The Case Management Program Supervisor is responsible for supervising staff in the Permanent Supportive Housing group. They provide oversight of the day-to-day operations of the program and ensures a smooth flow of work processes.

ESSENTIAL DUTIES AND RESPONSIBILITIES :

  • Monitor occupancy and vacancy of units and ensure occupancy is at an optimum level.
  • Coordinate and monitor daily operations of case management staff, ensuring adequate staffing and coverage.
  • Responsibilities include hiring, conducting regular performance evaluations, coaching, reviewing requests for time off and schedule changes, discipline, and termination of assigned personnel with support from Permanent Supportive Housing Manager.
  • Plan and facilitate regular staff meetings.
  • Maintain effective communication, with direct reports, supervisor, tenants and property management staff.
  • Oversee appropriate management and maintenance of client records system. Input and maintain accurate records of client's progress in working toward individualized housing goals using MS Word and Excel and HMIS.
  • In partnership with the Resident Services Program Manager, conduct partner and internal program, file, and data audits.
  • Assist in development and implementation of new policies and procedures and processes as needed.
  • Provide training, assistance and guidance to staff who are assisting clients in working toward the goals of healthcare connections, income enhancement, obtaining and maintaining permanent housing, and developing self-sufficiency.
  • Provide assistance and guidance to staff who are assisting individuals in completing housing assessments and developing goal driven plans.
  • Work with internal and external housing providers to assure coordinated and streamlined transitions out of homelessness and into housing.
  • Conduct follow up activities consisting of progress checks, phone, and mail contacts.
  • Adhere to all state and federal privacy and security regulations applicable to the program, and to CCC policies and agreements regarding confidentiality, privacy, and security.

This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.

DESCRIPTION OF OTHER DUTIES :

  • Oversee and facilitate events as needed.
  • Provide assistance to the Permanent Supportive Housing Operations Program Manager as needed.
  • Complete required documentation in a timely and accurate manner.
  • Participate in compliance, quality assurance and quality improvement activities as directed.
  • Attend appropriate community professional meetings trainings, and events.
  • Take a hands on approach with team members when difficult client related issues need resolution.
  • Perform other duties as assigned.

SKILLS AND ABILITIES :

  • Strong time management, follow through, and organizational skills.
  • Ability to maintain accurate records and necessary paperwork.
  • Preferred knowledge of landlord-tenant law and fair housing law.
  • Familiarity with Microsoft Outlook, Word, and Excel.
  • Knowledge and skills in working with individuals challenged by homelessness, health, mental health, and or chemical dependency issues.
  • Ability to successfully interact and communicate clearly with a wide variety of people including landlords, partnering agencies, supervisors, and co-workers.
  • Ability to be creative, innovative and a progressive problem solver.
  • Demonstrated ability to work independently and as a member of a team.
  • Demonstrated leadership ability.
  • Ability to learn and apply training instruction.
  • Ability to set and maintain professional boundaries, and to de-escalate volatile situations.
  • Demonstrated knowledge of issues related to individuals with multiple barriers to housing.
  • Ability to understand and follow oral and written instructions.
  • Demonstrated expertise in assisting individuals to develop individualized plans and providing on-going support as they work to achieve goals.
  • Familiarity with HMIS.

MINIMUM QUALIFICATIONS :

  • High School or GED required. Bachelor's degree in a relevant field preferred.
  • 2 years of experience working in supportive housing, behavioral health, social services, or related environment with 1 year supervisory / lead experience.
  • Must possess a current driver's license, pass a DMV background check and be designated an acceptable driver as outlined in Central City Concern's Fleet Safety Policy.

Must pass an initial drivers training and then pass an annual recertification.

  • Must be able to demonstrate excellent computer skills.
  • Must pass a pre-employment drug screen and background check.
  • Will be required to carry an agency cell phone for work use. Cell phones will be provided by Central City Concern.
  • Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects.
  • Must be able to climb stairs several times a day.
  • Ability to adhere to Central City Concern's drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Work Place Act of 1988.

  • Applicant must be able to work in all CCC buildings. Some CCC buildings have restrictions due to criminal background and / or active probation and parole.
  • Must adhere to agency's policies, including comply with drug and alcohol free workplace.
  • Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, cultural backgrounds, life-styles, and sexual orientations and treat each individual with respect and dignity.
