This position involves assessing patients' health conditions, administering medications, and collaborating with interdisciplinary teams to ensure comprehensive care.
Your expertise will be instrumental in promoting the health and well-being of patients recovering from surgery, managing chronic illnesses, or requiring assistance with daily activities.

Monday through Friday 8a-4p. No call or weekends required.
Responsibilities
- Conduct thorough assessments of patients' health status in their home environment.
- Develop and implement individualized care plans based on patient needs and goals.
- Administer medications and treatments as prescribed by physicians.
- Monitor patients' progress and maintain accurate medical records.
- Educate patients and their families about health management and care practices.
- Coordinate with interdisciplinary teams to provide holistic patient care.
- Provide emotional support and guidance to patients and their families.
Requirements
- Current and valid RN license in the state of practice.
- HomeCare Homebase Charting Experience Required
- Minimum of 1 years of direct home health experience, within the last 3 years.
- Strong clinical assessment and decision-making skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Basic life support (BLS) certification required.
